A gentleman 43 yo came to a senior homeopath for painful corn on his palm since one year. The senior homeopath asked me to give a single dose of Antimonium Crud 200. To my surprise the corn disappeared within 3-4 days. This case proved conclusively to me the power of specifics. i am a great admirer of prescriptions based on totality still specifics have their own space.

This is an old case and the lady is complaint free since 2 years. A 28 yo unmarried lady came to me with complaint of stiffness and pain in her lower extremities. She has white swelling on her knees. She has more stiffness in the morning and her pain are worse after movements. She has taken lot of allopathic medicines for her pain which has stopped working now. She suffers from indigestion during menses. She gets aggravation in her pain after exposure to cold weather. She gets profuse and early menses. She said that her body pain has started after she had chikunguniya few years back but recently it has reached to unbearable levels.
This case was done on the basis of characteristic materia medica. The repertorial analysis is done only for presentation purposes.
She was given Rhus Tox 200 bd for few days which didn't give her any relief. Nux Vomica 200 provided some 25% relief. She told me that she gets burning in her palms during menses. She was asked to take Kali Carb 200 followed by Carbo Veg 200 which provided good relief to her on all counts. i would again like to mention that homeopathic prescribing involves lot of subjectivity. It also demands immediate attention for acute flare ups. Therefore being methodical always and rule abiding is not possible in real conditions. All cases are being shared as it is without decorating in any way for presentation.

A 30 yo unmarried male patient comes to me for his abdominal pain on flare ups. He generally does well on nux vomica, calc, lyc depending on his other presenting symptoms. Once he reported that he has got flare up in tinnitus. It worsens terribly during masturbation. He said that since few days it is even persisting after masturbation. A single dose of Digitalis 200 provided him immediate relief in his tinnitus.

A 19 yo girl came to the ngo clinic. The girl seems to be from an affluent family. She skillfully avoided disclosing the source who recommended her. She was speaking very mildly. She said that she has been just operated for her rectum fistula. She gets headache and vomitting after she returns from her college. She told that she can't bear any contradiction and gets very angry if she faces any. She is also irritated at home for trivial matters. She doesn't have any contention of mind. She is little bit obese.
She did well on Nux Vomica for her vomitting and headache. She also responded later to calc and then lyc when there was a relapse. She got some warts on her hands. She was given Thuja. Warts decreased in number but started itching. She was given Sulphur. Itching subsided but warts remained. She was given Nitric Acid with which all her warts dropped. When she came for the warts episode she reported that her headache and vomitting has been cured.

A boy 17yo was brought to me by his father. He looked very weak. He had an ulcer on his lip which looked like a fever sore. His father handed over a file to me and said "He was hospitalized for few days after he fainted in his college." The file said that the boy was suffering from enteric fever. His father said that had fever even after discharge from the hospital and was given allopathic medicines. He feels weak, has haeadache and less appetite. i gave him Nux Vomica 30 bd for 7 days. Then after clinic hours i made the following analysis. i was all set to give him Arsenic Alb 30 when he visits me after 7 days. He didn't turn up on seventh day so i thought that he should have sought treatment somewhere else. On eightth day he appeared and said that he is alright now. No fever, no weakness, no lip sore, good appetite.

A 44 yo came to me with a complaint of chest pain. She went to a cardiologist for this complaint earlier. The cardiologist advised her hospitalization. It would cost her lots of rupees so she didn't do that.
While narrating her complaint she was pressing her left breast and making some other extravagant expressions. The first rubric was applied for her overt expressions at a public place (the ngo clinic is a virtual public place). The second rubric for her perception that there is a serious problem with her heart. The third one for her fear that she will suffer a heart attack. Hyos 200 single dose was given and she reported back after 7 days that there is no chest pain.

A 75 yo lady was brought to me by the volunteer who looks after the upkeep of NGO clinic. She told me that this old lady stays near the clinic. She is not given a single penny by her children and she doesn't have any source of income. So i should not charge even the nominal fee to her. The old lady started weeping. She said what a shame it is that those who are brought up as children do such things in our old age. She said that she has a constant discharge from her right ear. The discharge was not very thin like water nor very thick and viscous. It was slightly yellow. There was spasmodic pain in her ear. The pain was not continuous.

A 45 yo came to me with complaint of anxiety. He said that he always fears that something wrong will happen. He gets very anxious and tense and continuously remains in that state. It also affects his day to day working.

Patients come to me with a complaint of prostatorrhoea. They suffer from weakness due to loss of prostatic fluid while passing stool or at other times without erections. Selenium is a specific remedy for this complaint. i have used selenium 6 effectively in at least two cases. Other remedies with two occurences in the following chart are also quite effective. A lady brought her 6 month old kid to the clinic and sought treatment for haemangioma. The kid had haemengioma on his head and back. The allopathic doctor presiding as the chief of the clinic asked the lady to take her son to the government hospital. She said that she had been there but they denied any treatment. i intervened and said that let me give him a remedy and see how that affects over 7 days. A 40 yo lady came to me with a complaint of swelling on her right hand. She said that she feels very irritated at home and is utterly upset. She has many complaints against her hub. She has cramping pain in her legs at night. She said that her knees are also much painful.
She was asked to take Sepia 200 bd for 2 days followed by Natrum Mur 30 bd. She was asked to report after a week. She reported after a week that there is good relief in her pain. All swelling on her hand has gone. Still she gets cramping pain in her legs but that is much less now. She is also feeling quite well at mental level. i told her that look happiness can't be earned through remedies. Whatever short term relief the remedy has given should be used to develop a healthy attitude that will ensure consistent practical happiness. She is asked to take Natrum Mur 30 once in two days and report back after 2 weeks.
The repertory may create some confusion. But if one reads Boericke materia medica one can find that Sepia and Natrum Mur are the indicated remedies here. i many times ask patients to take one remedy after another and i want to acknowledge here that this dosing many times works very well.

A 35 yo married gentleman came to me with the complaint of hypertension. When I checked his BP it was 180/120. I said man what are you doing? Why your BP is so high? He said that this is his normal. It is always like that. I asked him whether he has any symptoms due to his high BP. He said no. He gets breathless after exertion. I told him to mind his life style as there is a possibility of blockages in his heart. He should cut his fat intake, no red meat and very less oil. When asked about any problems that he might have in his life he said that he always remain happy and doesn't take any tension. He even forgets to take his allopathic BP medicine at times. I said that he should take his allopathic BP medicine religiously and explained that any lapse may prove fatal for him. He was given Lycopodium 200 bd for 2 days followed by Belladonna 200 bd for 5 days. Then i did the following analysis and was waiting for a week. He reported back after a week. He said that there is relief in breathlessness. When I measured his BP it was 160/110. I gave him a single dose of Sulphur 200 followed by Belladonna 200 bd for a week.

A 18 yo girl came to me for treatment of pimples on her face. They erupted suddenly after she applied an antibiotic cream on her face. The girl has a complaint of severe dysmenorrhoea on the first day of menses. She is a mild and affectionate girl. She is thirstless and even returns almost 70% full 1 litre bottle from her college. She prefers open air and cold surroundings. She complains of constipation which started recently. The girl has done well on Pulsatilla 200 earlier. Puls reduced her dysmenorrhoea drastically. Looking at the complaint of constipation I opted for Natrum Mur. She was given few doses of Natrum Mur 200 which did nothing for her. Then a single dose of Pulsatilla 1M was given followed by sac lac which reduced her pimples by 70% over 10 days. Even though the repertory throws up many remedies, the differential study of Materia Medica definitely helps for reaching the right remedy. It is also true that repertorization helps in reducing our bias for our favorite remedies.

That was an exceptional day at the clinic. The clinic that offers allopathic and homeopathic treatment didn't have presence of any allopathic physician. It is easy to treat old patients you have their medication schedule on their records. Of course, treatment of new patients demands close attention for making diagnosis.
A mother brought her 8 yo son. The boy had swelling on his cervical lymph nodes and he was also having fever. The boy was restless while sitting. He lacked focus of mind while answering questions addressed to him. I decided to give him belladonna 200. The remedy was given bd for 2 days and then od for 4 days. Amox and para was also given. When allopaths are present they write prescriptions for new generation antibiotics as clinic doesn't hold any stock of expensive medicines.
This is a weekly clinic. I did the analysis for the case for verifying the remedy given and to decide about the next remedy. I was having pulsatilla in my mind for the second prescription. I was quite curious about the outcome of the remedy when I went to the clinic after a week. I discussed the case with an allopath who said that the boy needed new generation antibiotic. When the mother brought the boy, I was pleasantly surprised to see that the boy got relief in fever and cough. There was a substantial reduction in the swelling of lymph nodes. The mother said that the boy won't take the bitter medicine so give him only your sweet pills!!

People generally don't know that homeopathy can give good results in the treatment of psychological disorders like mania, insanity, delirium etc.
I used to think earlier that remedies like Natrum Mur and Ignatia should do well if there is history of grief. Similarly Staphysagria, if there is history of humiliation or mortification.
A person who was coming to me for the treatment of himself and his family members once saw that I was dealing with a patient with psychological disorder. A 73 yo gentleman came to me with a complaint of itching and burning on hand. He was doing his morning yoga in a park. Suddenly a small monkey jumped onto his hand. A 46 yo lady came to me with a complaint of watery and gurgling diarrhoea in the evening. The lady generally does well for her complaints on Lycopodium. But this time it was an altogether different complaint. She had some heavy food on the day of festival. She started having loose motions on the second day. Her condition was not responding to the allopathic medicines. She was hospitalized in the night. She was discharged from the hospital still she continued with watery and gurgling diarrhoea. She passed 25 loose motions after her discharge from the hospital. i decided to go for specific. i gave her Aloe 200 (and Nux Vomica 200 to be taken with a gap of hour). The remedies were to be taken bd for few days. She reported very next day that her complaint was relieved with the very two doses she took last night. She complained of calf pain and asked remedy for the same. She also complained of severe bitter taste in mouth. A constitutional remedy is the one that is worked out on the basis of core traits of the patient's personality. When the constitutional symptoms are adjusted for the presenting symptoms we get the indicated remedy. The constitutional approach is not supported by the Organon of Medicine. Organon says that one should take only the changed symptoms for prescribing and not the constitutional symptoms. In my opinion the constitutional mind symptoms act as concomitants and they do play a significant role for individualizing the patient. It is also true that if we try to take only the changed symptoms we have pathology specific symptoms which are not that effective for selecting a homeopathic remedy.
Let me explain the working of this approach with the help of a case. This is a case of a 77 yo gentleman who has been under my treatment since 5 years. The following are his constitutional mind symptoms collected through personal interaction with the patient and his relatives.
He jests over matters in the course of enjoying a festival or a get together.
He presents himself to others in the best possible manner ie eulogizes himself for anything and everything that he does.
He focuses on dark side at each achievement of his family members. Even makes moments of celebration mournful.
He takes pride in everthing that he has, gives excuses for everything that he doesn't have and magnifies his ego.

He got fever 5 years ago and was undergoing allopathic treatment. He lost his appetite in the course of allopathic treatment. He also had alternating diarrhoea and constipation during his illness. He also developed severe insomnia. He regained his health after few doses of Lycopodium 200 and repeated doses of Nux Vomica 200. This happened about 5 years ago.
4 years ago he developed severe itch on his hand which responded very well to a single dose of Sulphur 30.
Around same time and later on also whenever he suffered from vertigo in the morning or diarrhoea after eating irregularities he responded well to Nux Vomica 200.
Once he got sick after a quarrel broke out between his sons. He got immediate relief from Natrum Mur 200.
6 years ago he had purple spots on his hand which were treated by another homeopath with Lachesis 200.
So the constitutional approach helps but one should not ignore presenting symptoms. The constitutional symptoms give you the perspective but you have to decide for the presenting symptoms.

Rhus Tox is an ace remedy for stiffness where first motion is difficult. Where the pain is better by continuous motion. It is well indicated in affections arising out of injuries, falls, blows etc. Rhus Tox pains are better by warm applications hence it is generally considered as a winter remedy.
i use it liberally as these are quite common conditions. The person whose constitutional analysis is done at the following link did well on rhus tox for his stiffness of hands in the winter season. You can see that Rhus Tox doesn't appear in the top 20 remedies for his constitutional analysis. So motion and time modality can overrule the constitutional analysis at times. No need to say that homeopaths become very confused at times (or many times ) while making selection of the indicated remedy.

A 32 yo lady came to me with a chief complaint of joint pain. She suffered from severe anxiety. She also had phobia for crowds. She had profuse sweating at her palms. She was apprehensive even about insignificant matters. She passed stool every alternate day. She get very close to the physician emotionally while narrating her complaints. She said that all her pains are relieved during her menses. She had frequent complaints of profuse watery discharge from her nose.
Lycopodium 200 bd followed by sac lac provided overall relief in all her complaints. Her panic attacks reduced drastically and she no more has any fear of crowds. i asked son to come and collect the remedy after half an hour meanwhile i did the above analysis. i decided to for Pulsatilla as the changed symptoms pointed to Pulsatilla. Generally i am not comfortable with Pulsatilla for fiery and ambitious business people. But here i decided to deviate from my practice.

He bursts in anger at home and threatens his parents liberally. i gave him Hepar Sulph 200 bd for 2 days followed by sac lac and asked him to come after 15 days. He reported relief in suppurating eruptions after 15 days, but there was no relief in his oily skin. i gave him Natrum Mur 200 bd for 2 days followed by sac lac for 15 days. i adopt the same approach of totality of symptoms for fever cases also. If the modalities are decisive for eg if summer heat is the reason of fever i go for the remedies like bryonia, natrum mur, glonoine etc. Similarly if the fever is after overeating i go for Nux Vomica, Pulsatilla etc.
In other cases i give importance to mind symptoms when the causative factor is not identifiable clearly. Needless to say that one should give the remedy which takes best care of peculiar symptoms of the case.
This is a case of two kids of a lady who works as a home-maid. The lady earns money by working as a maid while her hub remains unemployed most of the time. He verbally abuses her in front of all.
Once the lady brought me her daughter 8 yo with a complaint of fever. She was having severe headache on the right temple.
i gave lycopodium 200 to the girl and she responded well. At other times when she had fever she responded well to Pulsatilla and Nux Vomica. Only one remedy was given at a time.
Her younger brother also reponds well to the remedies Nux Vomica and Pulsatilla for his fever.
i didn't give other remedies as they did well on the aforementioned remedies. i don't believe that only the selected remedy can provide the necessary stimulation to the vital force. i believe that other seemingly indicated remedies can also do well.
Initially i was in favor of antipyretics and antibiotics for fever but now i am quite confident of the homeopathic remedies for fever.

A 68 yo woman came to me with a complaint of bleeding from rectum while passing stools. She was full of fear after watching blood dropping and spreading. She said that her anus feels constricted and it is very difficult for her to pass stool. She had a malignant tumor in her breast which was surgically removed 4 years ago. She has chronic pain in lower extremities due to issues with her lower vertebrae of spinal cord. She has herniated disc so Rubric 7 should be most appropriate than Rubric 5 or 6. A 23 yo boy came to me with various complaints at various points of time. He has been under my treatment since 3 years. He was detected with having malignant spinal tumor five years ago. The tumor was removed surgically. Chemo and radiation was also given. He used to get swelling on his leg as the tumor was on his lower vertebrae. A 46 yo gentleman came to me with a complaint of pain in his lower abdomen. When asked about his work and personal life he said that he is under lot of stress at his office. He gets irritable due to the work load. His boss always assigns heavy work to him. He hardly gets any leave. He also said that presently he is under a month's leave. When i asked him as to how he could manage such a long leave he said that he applied for 15 days leave and then extended it on medical grounds (fake). A 21 yo girl came to me with a complaint of painless eruptions all over her body. She also had eruptions on her face. She is a homely girl. Her family is looking for a suitable match for her. She doesn't have any career plans and is happy to be a house wife. She sweats a lot and can't bear heat. Earlier when she came for her complaint of piles she did well on calcarea carb. She was given Natrum Mur 200 single dose for her eruptions and she reported good improvement. Of course Puls, Phos and Sulph will be given due consideration and checked for their indications on her future visits.

A 47 yo lady came to me with the complaint of numbness in her extremities. She is a mild mannered lady but she appears to be sad. The expression of suffering could be seen on her face. She has a strange habbit of laughing very loudly suddenly in the course of discussions. She said that during her last menses she has too much pain. She rushed to an allopathic doctor and took a pain-killer injection. This has never happened before. She gets profuse menses. She gets her menses irregular too as probably she is reaching her menopause. She is a light sleeper and her sleep gets disturbed by slight disturbances. She is always busy doing household work. She did well on the remedies like Sepia, Calc Carb and Phosphorus in the past. Calc helped her for her sleep and it also reduced her menstrual flow. Sepia was given for her acute headache and some other acute issues. Phosphorus too worked on it's indications at other times. This time i decided to see how Cocculus works as this is not my favorite remedy. i could have given one of the earlier remedies but i opted for cocc due to the latest complaint of dysmenorrhoea. To my surprise the lady got relief for her numbness on the very first dose of Cocculus 30. Repertorization does enhance our outlook of remedies and the lovers of Materia Medica can also use repertory for writing more effective prescriptions.
